How to REMOVE spots on SKIN ?

HI, I just got the program, and I am trying to use the Brush which is over the portrait , to remove some spots on exposed skin not on the face. According to the instruction manual for MACS, it mentions pressing the LEFT MOUSE while you use the brush.
This sounds like a mistake in the manual if it is for MACS,, or else, I don't understand how to do this ( probably) simple task.

Would someone please help a newbie out and please explain how to remove spots on skin that are not on the face?

If the skin to retouch is a big area, I would go to Detect Skin (shortcut S) and brush those areas of skin to be included. After that, go to touch-up (short cut T) and "clean or airbrush" the skin.
